Is this guy interested or not?

I met a guy online and we had instant incredible chemistry. It's long distance, but we talked constantly and became emotionally intimate very fast. He treated me wonderfully, and was very open with his feelings. He talked about the future, and told me how lucky he felt all the time. We met up every two weeks or so and things were fantastic in the beginning: great , lots of affection, lots of caring. About a month ago, it's like a switch flipped. He got in a bad accident and it's like it changed his feelings for me somehow. He still talks to me all the time, but the affection and the intimacy are gone. We used to be really sweet to each other but now my sentiments go ignored. When we see each other now, he doesn't seem interested in being physically intimate, and when we are, he doesn't care if I finish. What the hell is going on? My feelings haven't changed, and he still talks about the future and says he's exclusive to me, but something is definitely off. Please help, it's breaking my heart daily, but I'm scared to talk to him because I don't want to scare him away by seeming too needy long distance.
